Our process involves:

Initial consultation - having an in-depth initial consultation in your home (to conduct a comprehensive assessment based on your needs)

Care Plan - developing a personalised care plan to outline the supports and services you require, collaborating with your Home Care Package manager or family to ensure that our plan aligns with your goals and funding

Implementation of supports - This can include intervention to work towards your goals, trialling and sourcing assistive technology and equipment, ongoing appointments if needed.
We support clients with Home Care Packages, White and Gold Card Veterans, Medicare (Mental Health Care Plans and Chronic Disease Management Plans), Private Health funds or privately paying clients.
